Are you presently taking prescription medications?
Can you afford them?
If not, we may be able to help you!
We will work with you and your doctor to help you obtain free or low
cost medications through special programs offered by many of the drug
companies.
You must bring the following with you:
Current proof of income, such as annual W-2 forms, Social Security statements,
paycheck stubs, and last 3 month’s bank statements
If married, both husband’s and wife’s proof of income
Prescription medications that are current –
in their original containers
There is a non-refundable processing fee of $5
(per person seeking assistance).
NOTE: There are some prescribed medications
that are not covered under this Medication Assistance Program (MAP).
We will not know which medications are not covered until we check your
medications and the pharmaceutical company. There is no assistance available
for “over-the-counter” drugs. There is no assistance available
if you receive free prescriptions through Medicaid, Veterans’
Benefits, HMO, insurance, or other programs.
